Application Packet - This packet provides all of the forms needed for Midland's 2024-2025 student registration process in one handy download. It contains a copy of each of the following:

Student Application - Contains general demographic information about the student, parents, and church affiliation.

Financial agreement - Info about the parents and/or those responsible for paying school tuition. Specific dollar figures are included for elementary, middle school and academy.

Emergency info - This contains emergency contact information, a consent-to-treat agreement, and a student-release section.

Health inventory - This contains information on health history and a medical evaluation of the student. This form is required for new students OR for students entering 4th grade and 9th grade.

Image release - Allows a parent or guardian to indicate if their child's name and/or photo may be used by Midland for advertising, news releases, website, etc.

Request for Student Records - Form to be given to a previous school requestion that student records be send to Midland.

Pre-arranged absence - This form should be filled out when a student (grades 9-12 only) will miss school for reasons including doctor appointments, attendance at a funeral, personal illness, or unique family situations for which prior arrangements have been made.
